online process (this can be done at the
time of registration or through the link
sent to the e-mail address provided by
you at the time of registration). This
must be completed by the first game. |

Completed/signed waivers
must be submitted for all players no
later than the day of the first game.
Please include your team name and age
group on the waiver form. |

Failure to
comply with the above may result in
removal from the league or your team may
be required to forfeit games until
everything is in order. A score of 3-0
will be the posted result for all
forfeits. |

Refund Policy |

| LEAGUE TEAMS |
|
A full
refund will be given to teams
withdrawing from the league by the
posted deadline. Teams withdrawing after
the deadline has passed are subject to a
50% refund ($100) and also a 50% credit
($100) to enter a future session within
the 2008-2009 season. Deposits are
non-refundable to teams withdrawing
within 1 week of the start of the
session. FSA reserves the right to
handle all refunds on an individual
basis. |

| TOURNAMENT TEAMS |

refund will be given to teams
withdrawing from the tournament by the
posted deadline. Teams withdrawing after
the deadline has passed are subject to a
50% refund. Fees are
non-refundable to teams withdrawing
within 1 week of the start of the
tournament. FSA reserves the right to
handle all refunds on an individual
